Reflection on failures and mistakes.
It’s never easy to admit you’ve made a mistake, but it’s an essential step in learning, growing, and improving yourself. You can only learn from a mistake after you admit you’ve made it. As soon as you start blaming other people, you distance yourself from any possible lesson. But if you bravely stand up and honestly say “This is my mistake and I am responsible” the possibilities for learning will move towards you.
Mrs J was not well and GP was called to see her. I forgot to inform her daughter who is the next of kin. She was very upset that she was not informed that her mother was not well. I apologise for not getting in touch with her as I forgot to do so. I reassured her that it would not happen again.
